Hey, everybody. Welcome to today's lessons regarding legal responsibility in medication administration. This lesson discusses medication errors, how to report them, and why it is important to do so, as well as follow-up actions. It is very important to understand these concepts as proper documentation ensures patient safety. Medication errors can occur for a number of reasons and in various ways. One way to think about medication errors is that they are the opposite of the rights of medication administration. Wrong patient, wrong drug, wrong dose, wrong route, wrong time. Wrong patient means administering medication intended for one patient to another, often due to mix ups in patient identification. This is why it is critical to verify the patient through two identifiers, such as name and date of birth, prior to administering medication.
